    Oral Cancer Surgery

    The cell causing cancer is carried through the watery fluid called lymph and enters into the lymphatic system. That is the reason they appear and attack first the lymphatic nodes in the neighboring area. Oral cancer have chance to spread to lungs, neck and other internal organs.

    Causes :

    Though it is accepted universally that people who smoke and drink are prone to get cancer of all types, still this theory remains confused in some aspect. Any person who does not even smoke at least once has every chance to get oral cancer. But doctors believe that tobacco and smoking is the main cause for oral cancer. Heavy smokers are more prone to get mouth caner and lung cancer. Similarly drinking alcohol can induce cancer. Exposure to direct sunlight can cause skin cancer and lip blisters. For people who have family history of cancer or head cancer have more chance of getting oral and neck cancer than others.

    Symptoms :

    You have to look up for early signs in case of oral cancer. White patches inside the mouth and in particular on the lips should not be ignored. These patches may become malignant. Red patches of (erythroplakia) are another symptom of oral cancer.
    Your mouth or lips may be bleeding, formation of sores, difficulty in swallowing, dental problems, lymph in the neck and earache are other symptoms.

    Diagnoses :

    Your doctor will examine the oral cavity and check for red and white patches in the lips and mouth. He would also look for lymph nodes in the neck to see any swelling has developed. He would then take out a sample tissue for biopsy. Local anesthesia will be given for removing sample from the mouth. The biopsy result will give the clear picture of oral cancer.

    Treatment :

    Your doctor should first analyze the stage at which the cancer has advanced, before sorting out the right method of treatment. He would then consider the size of the tumor, its position and how far it has spread. Taking a dental X ray and CT scan or MRI scan of the affected part is advised to proceed with treatment.
    Surgery will be done for removing tumors of small size and lymph nodes on the neck. Radiation therapy is given to destroy the cancer causing cells in the oral cavity. It would gradually shrink the tumor to small size which can then be removed by surgical process. Chemotherapy is used for improving immunity and for killing cancer cells. You have to take several shots of injection while taking chemotherapy.

    Cleft lips are congenital in nature meaning inherited by birth. It refers to the condition of opening of lip or lips into the roof of the mouth due to incomplete development. One out of every 1000 babies have cleft lips and this condition is considered to be one of the major birth defects. However cleft lip can be treated easily with minor surgery done to the child. The main reason for the formation of cleft lip is due to impartial closure of lips during the first trimester of pregnancy. Sometimes the cleft lip may extend up to the corner of the lips or even till the nose. Cleft lip varies in size and may occur in combination with cleft palate.

    The condition of cleft lip can be categorized into three types. One is the cleft lip formation without cleft palate, second is cleft palate condition without cleft lip and thirdly cleft lip and palate occurring together. It can occur on any corner of the mouth left or right and is more common among boys than girl babies. Similarly the occurrence of cleft palate is prominent among girl child

    Though the formation of cleft lip or palate can be detected while the mother is pregnant, still nothing can be done until the child is born. The exact reason for the occurrence of cleft lip is not known. But it is genetic in nature and hence cleft lip and palate is formed due to gene mutation during the formation of zygote. It may also be caused due to virus infection or intake of strong drugs during pregnancy.

    It's no secret that you need to care about your teeth from early childhood, and even baby teeth’s problems require quality treatment by a dentist.

    The difficulty in treating children is that they don't like to stay in one position for long time; unfamiliar surroundings often scare them and a child could simply refuse to open his mouth. Visits to the dentist are often stressful, both for children and their parents.

    The use of modern medications permits to put a child to sleep for the time it takes the dentist to thoroughly treat the damaged teeth and make the very process of treatment almost imperceptible.

    The Pediatric Department of the European Dental Center uses the safest medication to date for treatment of children under medication sleep. It smells good, and breathing it, the children don’t experience any discomfort. The medication acts very quickly – just after the first breath patients fall asleep, and within 15 minutes after anesthesia wears out, little patients already are able to make decisions and move about independently. This medication does not affect the brain, heart, liver or any other major organs and systems and therefore is suited ideally for routine anesthesia in children.

    Indications for dental treatment of children under the medication sleep


    Medical treatment of children under medication sleep is appropriate when a child requires extensive dental procedures. For example, it would be suitable in case of multiple cavities, which would require several visits to the dentist. A child could accumulate negative emotions from these visits, and elements of violence to ensure dentist’s work could easily lead to stomatophobia. For some people, this panic fear of dentists remains for the rest of their lives. It is difficult for a dentist to do quality work under these conditions. Under medication sleep, the treatment can be completed in one visit, and child's negative emotions will be minimal. Moreover, the sedation helps to reduce salivation, which also makes the treatment better and more efficient. Treatment under medication sleep is also advised in case of allergy to local anesthesia.

    On the day of treatment, it is necessary:


    - not to feed your child at least 6 hours prior to your appointment time,
    - not to give drinks to your child at least 4 hours (6 is even better) prior to the appointed time of treatment.

    A. Cleft lip is not always associated with cleft palate.

    Q. What can be done for my child born with a cleft?
    A. Surgical repair of the cleft can be successfully done restoring appearance and function to normal. Most of the babies born with cleft have nasal deformity, so long with cleft surgery, nose correction is also done. Many of our patients have gpt excellent results.

    Q. Why do cleft lip and palate require many surgeries? Is it not enough to do once?
    A. Its is not enough as the doctor has to decide for the child as the functional treatment always precedes the esthetic correction and has to be constantly revised as the child grows.

    Q. Will my child look normal after surgery?
    A. Not immediately after surgery because of the swelling in the lip region and the scar appears red. The scar takes about 6 - 12 months to disappear to give a pleasing look

    Q. How many days should my child be in the hospital after surgery?
    A. The child is hospitalized for maximum of 2 - 3 days.
    Q. Can parents stay along with the child in the hospital?
    A. Parents can stay and take care of the child in the hospital. The details about the stay can be cleared from the hospital staffs.

    Q. How should I feed my child before surgery?
    A. If the child’s lip is incompletely split and the palate is intact, breast feeding or nursing bottle can be used. Regarding feeding please as the the doctor in charge as the child is always placed under 6 hour fasting prior to surgery.

    Q. How will my child feel about having cleft?
    A. Insecurity in infants is due to parents’ depression, child’s lethargy and irritability. Social support, self-confidence and trust might prove advantageous. So parents should have frank discussion about the cleft and should give proper guidance and encouragement

    Q. Whom should I ask concerning the cleft?
    A. Team of specialists- plastic surgeon, orthodontist, pedodontist, speech therapist, nurses work together. So you can contact any of these members or Balaji‘s Craniofacial center regarding the cleft.

    Q. Will my child speak like normal child?
    A. Child speaks very well after the surgical repair of the cleft. The improvement in the speech depends on the co-operation and encouragement shown to the child.

    A. You should always repeatedly encourage your child to speak. Child should be taught first a few words then small sentences and gradually trained.

    Q. What should I do to make my cleft child speak before surgery?
    A. You should talk, play and laugh. An attempt should be made to produce sounds clearly

    Q. When should I meet speech therapist?
    A. Not all the children need speech therapist. Only for few children speech therapy becomes necessary and speech therapist has to be contacted at 2-3 years of age.
